"ICE DAYS" PEE WEE HOCKEY LEAGUE

PEE WEE HOCKEY COMES TO CONYERS FIRST ANNUAL  SEASON

WHO:       7, 8 & 9 yr. Old  BOYS & GIRLS,  born on or after July 1^st, 2003,2004 & 2005.
WHAT:     HOCKEY LITTLE LEAGUES
WHEN:     TRYOUTS:   NOV. 13^th   for 7 & 8 YR. OLDS
               TRYOUTS:   NOV. 14^th   for 9 YR. OLDS
               SEASON:      NOV. 15^th   thru FEB. 6^TH, ‘13
WHERE:   PAVILION OLDE TOWN CONYERS
HOW MUCH:  Registration Fee = $225 + $25 for hockey jersey/socks for player to keep at end of season Total = $250  Cash, check, credit cards-all major
PAY : On-site, after tryouts, when player makes the team
REGISTERED:  by Dec. 1st, 2013  Reg. form email back or print &   mail

PLAYER’S  RESPONSIBILITIES:
-Must be able to skate forwards and backwards (practice inline roller blading)
-Must provide own equipment (excluding jersey/socks provided by HAID)
   -Helmet with face mask + mouth guard + athletic supporter
   -Shoulder, Elbow, Hip and Shin Pads
   -Black pants
   -Skates/Stick
-Ice Days will provide goalie glove, leg pads, and stick
-Must bring and provide birth certificate, certified copy
-Parent/Guardian must sign w/player
“Release of Liability Form”

OTHER:
-10 Games total w/championship + 4 Scrimmages
-VOLUNTEER COACHES NEEDED,
